My theory: That hollow is based off of Kaien Siba's soul and, thus, Kaien Shiba is the hollow ichigo that we always saw. It's Kubo enough to make sense.

Also, Masaki is a badass.

And that he transferred some of his powers to Masaki when he bit her so that when Ichigo was born, he had his powers.
